What disclaimer do we need as affiliates for FTC?

Just wondering what disclaimer do we need as affiliates for FTC? I watched one Joey's videos and the message in the video said to ask what disclaimer we need. Do we have some template that we can use? That would be super helpful.

This is about the extent of my disclosures most of the time. IMO as long as you have something on there. I usually have this on the bottom of my pages.
